Release Request Procedure for Toronto Stars Lacrosse

If you are enquiring regarding a release from a Stars Lacrosse organization you must follow the following procedure as per the Ontario Lacrosse Rules.

  1. Go to www.ontariolacrosse.com
  2. Go to Administration
  3. Go to OLA forms
  4. Open up Release Request form - complete and EMAIL only to the repdirector@starslacrosse.ca, if for some reason you cannot email, then fax to 905-477-3614, DO NOT MAIL a release from.  After the form has been received by the Stars, we have 72 hours to respond to you, in regards that we have received your request form.
  5. A meeting with the Stars is then set up, ether with a committee or at one of the organizations meetings, a clinic or wherever  the organization is able to meet with you to discuss your release request in a timely fashion.   
  6. After the meeting the Stars will notify you of the decision within 24 hours by email.
